A patient with knee pain who searches "physical therapy near me" may not know they can come without a referral. In Texas, that depends on the rules and on your therapists' qualifications.
Texas Occupations Code ยง 453.301 allows a physical therapist to treat without a referral for up to 10 consecutive business days, or 15 with a doctoral degree plus a residency, fellowship or relevant certification. The therapist must have been licensed at least a year, carry professional liability insurance, and hold a DPT or have completed 30 hours of continuing competence in differential diagnosis. The patient signs a disclosure acknowledging that physical therapy is not a substitute for a physician's diagnosis and that insurance may not cover the visit.

Google Ads: direct access and specialties
LocaliQ does not break out physical therapy. In LocaliQ's 2026 Search Advertising Benchmarks, Physicians and Surgeons averaged $4.76 per click, a 12.43% conversion rate and $40.04 per lead, and Health and Fitness averaged $6.17, 6.94% and $67.36. We plan with the range.

Google's health policy for personalized ads restricts services that treat or manage chronic health conditions, so plan campaigns for chronic-pain programs without your own remarketing lists.

Automations that fit how a PT clinic works
- Referral intake. Faxed or emailed referrals are read for patient name, diagnosis, referring provider and orders, and a draft appointment request lands in the front-desk queue. See document data extraction.
- Direct-access disclosure. Patients booking without a referral sign the required disclosure online before the evaluation, and it is filed to their record.
- Direct-access clock. Each direct-access patient's business-day count is tracked, and the therapist is alerted before the 10- or 15-day limit so the physician handoff happens in time.
- Progress notes to referrers. When a progress note is signed, a summary goes to the referring physician, which keeps referrals coming.
Each of these handles protected health information. HIPAA requires written assurances from any vendor that handles it for you, including automation platforms and AI providers. We choose tools that sign, or keep patient data out.

Honest limits
If your clinicians do not meet the Texas direct-access qualifications, do not advertise direct access; advertise specialties and fast scheduling instead. If you receive fewer than ten referrals a week, a shared inbox and a checklist may be enough. And if your EMR already handles intake well, connect it rather than replacing it.
